Appeal for Financial Aid Reinstatement

Special Circumstances 2012 - 2013
Complete this form if you are a dislocated worker (lost your job), or have gone through other significant changes in financial circumstances that warrant a review of your financial aid eligibility. To complete the 'Special Circumstances' process, Western must verify your current financial aid information. If you have not already completed the verification process, please print the appropriate verification form, complete, and include the additional verification info with your 2012-2013 Special Circumstances form.

Along with the Dependent Verification Worksheet, you must also include the following documents:
- Student's 2011 Federal Tax Return Transcript from the IRS (please SIGN)
- Student's 2011 W2's
- Parent's 2011 Federal Tax Return Transcript from the IRS (please SIGN)
- Parent's 2011 W2's
Independent Verification Worksheet 2012 - 2013
Along with Independent Verification Worksheet, you must also include the following documents:
