Complete the following form for all courses being offered as transcripted credit this school year. Include courses offered in both fall and spring semesters. 

Please only submit each course once (per term). Example: if you have 3 sections of Animal Science in the fall term, you only need to submit the form once with the total number of students combined in all 3 sections. Then do the same for the spring term as well. No need to fill the form out for each individual section.

If your school is on a 3-term schedule or a block schedule with courses starting after November 1, please submit course start date as follows:
Western Fall 2024 - High School Term 1 Courses (Indicate start date of September)
Western Spring 2025 - High School Term 2 and Term 3 Courses (Indicate start date as January)

After the Transcripted Courses are created we will arrange for the students to be registered online. Additional information will be sent out once the courses have been submitted and are ready for course registration.
